All documentation including before and after pictures, should be filled in the clients personal folder, following Data Protection Rules and Legislation.

Permanent Cosmetics Aftercare Advice

These forms are individual  after care instructions forms for Eyebrow, eye and lip enhancements, as well as beauty spot procedures and inform the client of the post procedure steps they must follow and for them to achieve optimum results from their permanent cosmetics enhancement procedure.

A list of the symptoms a client can expect should expect during the healing period are given and the correct use of post procedure homecare products are given.

These forms also describe the precautions to be taken to avoid infection and scarring.  They describe the considerations required for long term care of the procedure and will be given at the pre procedure consultation and again immediately following the treatment.

 

Pre procedure information:

Sensitivity Test Topical Plaster Method Instruction – Document 1

This is an instruction form for testing sensitivity to pigment and anaesthesia using the topical plaster method and is used if the client is not allergic to plasters method and it is not convenient for the client to come into the salon for a sensitivity test prior to her procedure.  The form is sent as part of the plaster testing.  A pre procedure form and a medical health questionnaire are also sent to the client in this circumstance.  The client is required to bring all the documents to the appointment.

A plaster testing kit contains:

  •       A plaster with pigment on
  •       A plaster with anaesthetic on
  • The sensitivity test topical plaster method instruction form

 

Sensitivity Test Topical Method Instruction – Document 2

This is an instruction form for testing sensitivity to pigment if the client is allergic to plasters and it is not convenient for the client to come into the salon for a sensitivity test prior to her procedure.  The form is sent as part of the topical testing kit. A pre procedure form and a medical health questionnaire are also sent to the client in this circumstance.  The client is required to bring all the documents to the appointment.

 

A topical testing kit contains:

  • 2 micro brushes
  • A container housing anaesthetic
  • A container housing pigment
  • Sensitivity test topical method instruction form

 

Medical Health Form – Document 3

This provides the practitioner with the clients medical background to enable then to assess if the client is contraindicated to the procedure.  It is also to assess whether a doctors consent to the procedure is required.  In some cases it may be necessary to identify if the client is taking any medication which may result in the treatment plan requiring adjustments. This form is to be completed by the client during the initial consultation or it is sent to the client with the sensitivity test kit. Check that the information has not changed immediately before the procedure.

 

Doctors Consent – Document 4

This consent form can be used if a client displays any medical conditions mentioned in the contraindications section of this manual that require a doctors consent.  The practitioner send s this document to the clients doctor requesting the doctor to consider the implications of his patient receiving the procedure, and to give his/her consent to the procedure.  It should be noted that many doctors charge for his service, so any cost should be passed over to the client to cover.

 

Pre Procedure Advice – Document 5

This form is designed to give the client pre procedure advice. It informs them that the result of the procedure are not instant and that a significantly darker colour intensity, a redness and swelling are to be expected post procedure.  The client will also be advised to avoid aspirin, ibuprofen and alcohol pre procedure.

Waxing and electrolysis are to be avoided close to the procedure, as well as eyelash curling treatments such as lash lift and lash and brow tinting if eyebrow and eyeliner procedures are performed.

Clients carrying herpes simplex virus and requesting lip procedures to use anti-herpes medications to prevent outbreaks.

General Consent and Procedure Permit – Document 6

This form gives the practitioner with documented evidence consent that the permanent cosmetic enhancement procedure can go ahead and that the client understands the nature and method of the proposed procedure.  It also confirms their understanding of the risks, side effects and any complications associated with the procedure.

This consent confirms the client accepts responsibility for the colour shape and position of the procedure.

Sensitivity tests have been offered and the client also confirms that this does not guarantee an allergic reaction will not occur.

The client consents to photographs both before and after are taken.  The client also confirms that they have been informed of agree to adhere to the pre and post procedure care instructions.

THIS FORM IS SIGNED BY BOTH THE CLIENT AND THE PRACTITIONER BEFORE THE PROCEDURE IS PERFORMED.

 

Chart Notes – Document 7

 

Think of chart notes as your treatment record.  It is essential in the permanent  cosmetic enhancement industry to keep concise records of the procedures undertaken with each client.  Detailed descriptions must be kept in order that you can satisfactory complete subsequent procedures.

This will be a requirement of your insurers.

The content of chart notes will be an accurate record of the following information:

  • Date of procedure
  • Client name
  • Service received
  • Colour of pigment
  • Needle configuration used
  • Anaesthetics used
  • Time spent on actual procedure
  • Final cost to the client
  • Method of payment
  • Date for necessary control appointment

In addition of the above, other remarks should be noted, such as the skin condition, skin reaction, sensitivity of the client, and any of any other reactions which may have occurred.  Finally any actions taken to remedy problems.  These chart notes should be referred to every time the client has an appointment, and as experience grows, you will be able to judge whether the methods and colours used were the best possible combination for each client.
