Module 1: Introduction to HIFU

What is HIFU?

HIFU (High-Intensity Focused Ultrasound) is a non-invasive cosmetic treatment that uses focused ultrasound energy to stimulate collagen production deep within the skin. It targets the SMAS (Superficial Musculo-Aponeurotic System) layer—the same layer addressed in surgical facelifts.

How it Works:

  • Ultrasound energy is delivered to precise depths, generating thermal coagulation points.
  • This stimulates tissue regeneration and collagen production.
  • Over time, skin tightens, lifts, and becomes firmer.

Module 2: Anatomy and Physiology

Layers of the Skin and Tissue

  1. Epidermis – Outermost layer; not directly affected by HIFU.
  2. Dermis – Contains collagen and elastin.
  3. Subcutaneous Layer (Hypodermis) – Fat layer.
  4. SMAS Layer – Muscle and fibrous tissue; HIFU’s primary target.
  5. Deep Muscle Tissue and Fascia – Affected with higher depth cartridges (4.5mm).

Collagen & Elastin

  • Collagen provides structure and firmness.
  • Elastin gives skin its elasticity.
  • HIFU boosts collagen via controlled thermal damage, triggering healing.

Module 3: Benefits of HIFU

  • Non-surgical skin tightening
  • Lifting of sagging skin

  • Contour enhancement (jawline, cheekbones)

Module 4: Tools, Equipment & Products

Main Equipment:

  • HIFU Device: Base unit and handpiece.
  • Cartridges (each with a specific depth and frequency):
    • 1.5mm – Superficial dermis
    • 3.0mm – Deep dermis
    • 4.5mm – SMAS layer
  • Ultrasound Gel – Coupling agent between probe and skin.

PPE & Hygiene Supplies:

  • Gloves, headband, sanitizing wipes
  • Disinfectant for machine and treatment bed
  • Cotton pads, tissue, alcohol wipes

Additional Tools:

  • Skin analysis equipment (optional but beneficial)
  • Marking pencil (to map out treatment areas)

Module 5: Client Consultation & Contraindications

Essential Consultation Steps:

  • Discuss medical history and lifestyle
  • Explain treatment process and expectations
  • Complete a consent form

Contraindications (Do NOT Treat):

  • Pacemaker or internal defibrillator
  • Metal implants in treatment area
  • Epilepsy
  • Severe or cystic acne in area
  • Pregnancy or breastfeeding
  • Recent dermal fillers or Botox (within 2-4 weeks)
  • Active infections, cold sores, or open wounds
  • Bleeding disorders or anticoagulant therapy
  • Severe diabetes (uncontrolled)

Cautions (Treat with Care or GP Permission):

  • Thyroid conditions (avoid direct treatment over the gland)
  • Rosacea or hypersensitive skin
  • Recent sunburn
  • History of keloid scarring

Module 6: Step-by-Step Treatment Protocol

Pre-Treatment Prep:

  1. Cleanse the skin thoroughly.
  2. Assess and mark treatment areas.
  3. Apply ultrasound gel evenly to target zone.

Treatment Steps:

  1. Choose appropriate cartridge based on area:
    • 1.5mm for fine lines
    • 3.0mm for deeper dermis (cheeks)
    • 4.5mm for lifting (jawline/SMAS)
  2. Begin at one side of the face and move systematically.
  3. Deliver lines of pulses in a grid pattern.
  4. Communicate with client regarding comfort level.
  5. Clean off remaining gel and apply a calming serum or SPF.

Post-Treatment Care:

  • Avoid heat exposure (saunas, hot showers) for 24 hours.
  • No vigorous exercise for 24-48 hours.
  • Hydrate well and wear SPF daily.
  • Avoid retinols and acids for 48 hours.

Module 7: Treatment Timings & Frequency

  • Single session may show some tightening.
  • Full effects take 2–3 months.
  • Maintenance suggested annually.
  • Some clients benefit from a second session 3–6 months later, depending on age and skin laxity.

Module 8: Troubleshooting & Side Effects

Common Side Effects (Temporary):

  • Mild redness or swelling
  • Tingling or numbness
  • Slight discomfort during treatment
  • Tenderness along jawline or cheekbones

Uncommon but Possible Issues:

  • Bruising (from treating too close to blood vessels)
  • Burns (from incorrect energy levels or poor coupling)
  • Nerve irritation (rare, temporary drooping if too deep or incorrect placement)

Legalities:

  • No current mandatory regulation in the UK, but:
    • Only trained professionals should operate devices.
    • Insurers often require a Level 4/5 qualification in skin or aesthetic treatments.
  • Risk assessment and treatment protocols should be documented.

Health & Safety:

  • Follow strict infection control.
  • Use CE-marked devices.
  • Patch test for highly sensitive clients if desired.
